Integrated Life Choices is seeking Host Home Providers (Independent Contractor), in Denver and surrounding areas, to provide support to persons with developmental and/or intellectual disabilities within their home to assist people to live the most fulfilling life possible in a family-like environment. This program matches individuals with Host Home Providers to provide support in the least restrictive environment. Supports include helping to learn new skills, become involved in the community, and to develop relationships and support to encourage independence. Host Home Providers provide 24-hour care/supervision including a safe and comfortable home environment, providing healthy meals, transportation, and support with daily needs. Host Home Providers also meet deadlines by completing paperwork, attending IPP meetings, assisting in developing programs, and completing assessments pertaining to the individual's program plan.
