Job Description

The Resource Case Manager located at Samaritan House is directly responsible for the service provision and locating shelter assistance for the participants who come in for assistance. This program serves families experiencing a housing crises in order to obtain short term shelter either through the Family Motel or via a contract motel in the community. Service modalities include Trauma Informed Care, Harm Reduction, Motivational Interviewing and Critical Time Intervention. Job duties include conducting needs-based assessments, community outreach, assisting with shelter navigation, connection to resources, attending meetings/trainings within the community, collaborating care with service providers and multi-disciplinary teams and provides transportation assistance.ESSENTIAL DUTIES AND RESPONSIBILITIES• Ensures that the family's voice is heard, and their ideas play a prominent role in programming when applicable.• Responsible for possessing an in-depth knowledge regarding program requirements, including the referral process, support services, and sheltering options.• The Case Manager facilitates the delivery of appropriate services for the households as indicated by relevant service modalities through an equity lens.• Demonstrates knowledge of family-specific resources, a passion for serving the family population and people experiencing homelessness, and the ability to contribute to the program-wide objectives of a diverse and dynamic team.• Supports team members by serving as a resource for analyzing and solving problems and staying abreast of current issues and theories within the field.• Attends orientations, trainings, education programs, staff meetings, community meetings, conferences and workshops as requested and applicable to meet the needs of the position. May provide training and mentorship to team members and community stakeholders regarding best practices in relevant service models and practices.• Clearly documents all client interactions along with required eligibility and demographic information.• Performs duties in a professional manner by maintaining the confidentially of all information and by participating effectively within and across teams.• Determine eligibility of potential guests through review of the Do Not Voucher list and National Sex Offender Public Website.• Divert families from the homeless shelter system when other options exist.• Documents use of motel vouchers and bus tickets.• Meets monthly with Denver County Human Services to reconcile vouchers and bus tickets.• Nightly communication with Family Motel Guest Services Coordinator and contract motel staff regarding room availability• Performs job responsibilities in accordance with the Social Work Code of Ethics.• Performs all other duties as assignedRequirementsMINIMUM QUALIFICATIONS• Bachelor's degree in human services, social work, or a closely related field or related experience.• One-year direct experience (paid or volunteer) working with people experiencing homelessness and mental health needs; bilingual helpful.• Knowledge of community-based resources for people experiencing homelessness.• Willing to obtain first aid, automated external defibrillator and cardiopulmonary resuscitation certifications and keep current.• Criminal and civil background checks within 30 days of hire.PREFERRED QUALIFICATIONS• Experience working with literally homeless population; families.• Crises intervention skills.• Ability to inform families that there are no openings for shelter if applicable.COMPETENCIES• Models core culture attributes of Volunteers Of America - Colorado Branch that include "AIRS" (Accountability, Integrity, Respect and Service).• Models and pursues with vigor Volunteers Of America - Colorado Branch three strategical critical virtues of HHS (Hungry, Humble, People Smart).• Possesses genuine empathy and compassion• Open to receiving and providing feedback; focused on conflict resolution• Adaptable• Ensures equitable and inclusive services.KNOWLEDGE AND SKILLS• Strong written, oral, and interpersonal communication skills• High level of competency working with Microsoft Office suite• Strong time-management, organizational and prioritization skills• Able to work independently and as part of a collaborative teamWORKING CONDITIONS AND PHYSICAL REQUIREMENTS• Some lifting is required, but should rarely exceed 25lbs.• The employee may be exposed to severe weather conditions.• The noise level in the work environment is usually moderate.• Ability to work at a fast pace to see maximum number families in time allotted.POSITION TYPE AND EXPECTED HOURS OF WORKEight (8) hours a week working Saturday and Sunday or Monday - Friday 5pm - 9pm 20 hours per week.
