Integrated Life Choices is seeking Host Home Providers (Independent Contractor) in Longmont, CO and surrounding areas, to provide support to persons with developmental and/or intellectual disabilities within their home to assist people to live the most fulfilling life possible in a family-like environment. This program matches individuals with Host Home Providers to provide support in the least restrictive environment. Supports include helping to learn new skills, become involved in the community, and to develop relationships and support to encourage independence. Host Home Providers provide 24-hour care/supervision including a safe and comfortable home environment, providing healthy meals, transportation, and support with daily needs. Host Home Providers also meet deadlines by completing paperwork, attending IPP meetings, assisting in developing programs, and completing assessments pertaining to the individual's program plan. ILC provides training to ensure Host Home Providers achieve success. Exceptional HHP candidates will be: Dedicated professionals looking to make an impact on others Innovative problem solvers who will create unique opportunities for the people we support Supportive individuals who want to encourage living fulfilling lives and self-advocacy Exhibiting integrity with every action, acting ethically and honestly at all times Professionalism when supporting others, treating them with dignity and respect Providing Service Around: Longmont, Erie, Lafayette, Broomfield, Brighton, Wheat Ridge, Thornton Requirements Host Home Providers must: Two years of experience in the Health and Human Services or related field is required Preference will be given to those with group home or supported experience.
