Facilitatesthe development, implementation and ongoing adherence to the stroke program protocols and therapies.Ensures the protocols and therapies become the standard of care followed withinthe facility. Collaborates with medical and hospital staff to providemultidisciplinary care to patients with a diagnosis of stroke. Rounds withphysicians on patients with diagnosis of stroke, reviews clinical data. Provideseducation and clinical expertise to care team, patients and families.Collaborates with physicians and staff in an effort to decrease length of stay.Identifies and integrates patient learning needs, abilities and readiness intothe Plan of Care. Works in collaboration with the nursing team to develop andimplement individualized teaching plans for patients and families with regardsto stroke diagnosis. Collects and analyzes stroke program patient data toassist with documentation, clinical care and improved outcomes. Participates inrounds and proactively responds to patient needs to improve patient outcomesand positively impact overall patient experience.
